Your inquiry is the first step in your patient’s rehabilitation journey. While we review your submission, please follow these next steps.
Your next steps
To expedite the evaluation process, please send the following documents to Admissions via fax at 404-603-4504 or by electronic transfer:
- A cover sheet with the name and phone number of the most appropriate hospital contact.
- Demographic/FACE sheet with date of birth, social security number, and next of kin name and phone number.
- Insurance information with policy and phone numbers.
- Auto or homeowners insurance policy information
- History and physical findings.
- Physician consultation progress notes from relevant medical specialties. (orthopedics, plastics, infectious diseases, etc.).
- Progress notes from the past two days.
- All operative and procedure notes.
- All imaging reports, including CT scans, MRI scans, X-rays related to injuries, and Doppler studies.
- Most recent chest X-ray and/pr KUB (Kidney, Ureter, Bladder) scans.
- All labs and microbiology reports with sensitivities.
- Current medication and administration records (MAR).
- Initial therapy evaluations and therapy notes from the past two days.
- Vitals, ventilator settings, respiratory status.
- Phone number for the nurses’ station and patient’s location.
Once we receive the patient’s documentation, we may perform an evaluation to determine if the patient is medically appropriate for Shepherd Center.
