Your next steps
- Ask your healthcare provider to make a formal referral for you or your loved one to Shepherd Center. We cannot begin the official referral process until this happens. This is the fastest way to get a patient evaluation started. Do not skip this step.
- Advocate for yourself. After you’ve reviewed your our admissions criteria and you feel Shepherd Center is right for you or your loved one, ask your case manager, social worker, physician, or discharge planner to submit a referral for you. If your current care team is unfamiliar with Shepherd Center, you can share with them our guidance for healthcare providers on making a referral . Or they can fax the referral directly to 404-603-4504.
Remember that you have the power to choose where you’re referred for inpatient rehabilitation.
In the meantime, our admissions liaison will review your inquiry and will follow-up if more information is needed while we wait for your formal referral. We typically respond to inquiries within 48 hours during business days.
